YOUR PURCHASE OR BID INDICATES THAT YOU HAVE READ AND AGREE TO THE
FOLLOWING TERMS AND CONDITIONS,
BEFORE PUTTING ANY ORAL APPLIANCE IN YOUR MOUTH YOU AGREE THAT:
General
You acknowledge, Timeless Smiles is a dental lab and not a licensed dentist. It is your
responsibility to seek medical supervision.
You acknowledge that you do not have loose, decaying or decayed teeth, periodontal disease
(gum disease), or loose crowns.
You are of at least 18 years of age and will take the impression yourself.
You are not pregnant or nursing.
Trays are only intended to be used by the person who made the impressions.
You will follow proper care instructions to prolong life of appliance. We are not responsible for
distorted, cracked, or mangled appliance due to neglect of care instructions. Any appliance
needing replacement will need to be sent back for proper inspection.
You acknowledge and agree that the total liability is limited only to the amount of the products
purchased from Timeless Smiles.
Whitening
Whitening gel ingredients: Propylene Glycol, Glycerin, Carbamide Peroxide, Carbopol,
Triethanolamine, flavoring. If you are allergic to any of these ingredients do not purchase gel
until you have consulted with your doctor
Prior to use, consult with dentist regarding whitening
If you experience any problems, consult your dentist
Supervision under a dentist is recommended while using any type of oral appliance and/or
whitening gel.
The trays you are purchasing are for whitening and only whitening.
Bleaching gel will only whiten the natural tooth, and not crown, veneers, fillings, bridges.
Sports Guards
With any participation in sports serious risk and injury are possible. Persons wearing above
mouth guard are solely responsible for any fees resulting in injury.
Acknowledge the risk of injury playing sports and that protective gear does not give 100%
protection.
The purchased oral appliance is only to be used by the person whom the impression was taken.
Hawley or Acrylic/Wire Retainer
Timeless Smiles is a dental lab, not a doctor’s office.
With any wire type retainer shifting can occur without proper doctor supervision.
Before placing said retainer into your mouth, you will make an appointment with your
orthodontist/dentist to have them adjust it properly and seat it in your mouth.
If you have a NICKEL ALLERGY you will notify us prior to fabricating your appliance so the
proper material can be used.
If you a nickel allergy and do not inform us, Timeless Smiles will not be held responsible for
negative reaction to the metal.
